Choose the format that suits you:

Both files are in English and contain the same fields. Add the seller’s details, your order reference and the products you are returning. Check your eligibility and the applicable deadline before sending the form.
The template helps you give the seller clear information. It does not send a message, book a parcel or establish a right to return an item on its own.
Find a Polish store on zwracamdo.pl
Enter the store’s name or website below. Zwracamdo.pl is a Polish-language service with return information for stores in its directory. The interface and generated documents are in Polish; the English downloads above are a separate option.
For stores with confirmed return contact details, the service can prepare a PDF and email text using the order information you provide. Review the address and current store policy before sending anything. If the generator is unavailable, use the store’s own return instructions or fill in the downloadable template.
With an account, you can save the return, add a carrier and tracking number, and receive reminders asking whether your money has arrived. Shipping-label purchase is available only for selected stores where that option is offered.
Send the prepared email from your own inbox. Zwracamdo.pl does not send it for you. Booking and sending the parcel are separate steps. Preparing the request does not require an account; saving it and receiving reminders do. See the zwracamdo.pl returns portal for details in Polish.
Fill in the right details
Start with the seller’s name and contact details. Check the parcel return address separately: a warehouse address may differ from the company’s registered office.
Enter your name, contact email, order number and delivery date. List the returned products and quantities clearly, especially if you are keeping part of the order. The template leaves room for any return reference the seller has supplied.
The reason field is optional. Only add bank details if you have agreed with the seller to receive the refund by bank transfer. Keep a copy of the completed form and proof that you sent it.
Notify the store and send the parcel
First check the seller’s return instructions and the rights that apply to your purchase. Send a clear notification identifying the order and the items concerned. Where you are exercising a right of withdrawal, you can write:
I withdraw from the sales contract for [products] in order [number], received on [date]. My name is [name].
Then pack the goods and arrange shipping to the confirmed return address. Keep the tracking number and proof of postage until the return is resolved. A form inside the parcel does not replace checking whether you need to notify the seller sooner.
Which deadlines apply?
For many online purchases from professional sellers in the EU, consumers have a 14-day cooling-off period after delivery to notify the seller that they are withdrawing. Goods generally need to be sent back within 14 days after that notification. Exceptions include certain personalized, perishable and unsealed hygiene goods.
These are EU consumer rules, not a worldwide policy or a rule for every purchase. Private sales, business purchases and a store’s additional voluntary return period can follow different terms. Questions before sending
Do I need this exact form?
No. For an EU statutory withdrawal, a clear statement can be sufficient; this template organizes the details. Check the seller’s instructions for the address and any reference needed to match the return to your order.
Does the form include a shipping label?
No. Arrange shipping separately. If you use zwracamdo.pl, a label can only be purchased where the selected store has that option available.
When will I receive the refund?
Under the EU withdrawal rules, the seller generally has 14 days from notification to reimburse you, but may wait for the goods or evidence that you sent them back. A submitted form or a reminder from zwracamdo.pl is not confirmation that the refund has been paid.
